The Planning in Partnership Program, in collaboration with other appropriate agencies, aims to support older carers of people with a disability. The focus of the program is to address changing carer needs that arise as a result of ageing and to assist carers in planning ahead to anticipate these changes. Assist ageing carers who are the primary carer of people with a disability. · Identify immediate, medium and long term respite and support needs, and highlighting issues that may arise when the Carer can no longer fulfill their caring role · Resource and empower Carers to identify strategies for meeting their expressed needs · Work collaboratively with case managers from care recipient focused agencies (e.g. Disability Services SA) to develop a Carer Support Plan that focuses on meeting the changing respite and support needs of the Carer over time and maximising continuity of care for the person with a disability · Establish and maintain a team of volunteers who will be trained and assisted to provide regular individual support to Carers who are participating in the program. · Arrange Carer support activities that enhance wellbeing and provide individual and group information and topics relevant to the target group · Periodic review of the Carer Support Plan to ensure that changing circumstances for Carer and the person with a disability are recognised and addressed Eligibility: Carers aged 65 years plus, or an Indigenous Carers 50 years plus, living in South or East metro area, caring for a person with a disability and currently not receiving any regular case management supports.
